About Amendments
All amendment requests must be submitted and approved by both parties before the Charter Holder can take action on or implement the changes requested. Amendment requests are submitted electronically through ASBCS Online by the charter’s authorized charter representative. Authorized charter representatives have received the following directions on how to access and use ASBCS Online:
The changes identified below require the charter holder to complete and submit an amendment request. The instructions for each request are linked below and any forms and templates identified in the instructions are located in the Forms/Templates tab above.
The Arizona Online Instruction Program Amendment Request is used to include the provision of specifically online courses or the provision of a comprehensive offering of courses that would constitute an online school under A.R.S §15-808, for the grades currently approved for the charter.
The Charter Holder Governance Amendment Request should be used if there is a change in officers, directors, members, or partners of the charter holder. This is the only form required if the charter holder and school governing body are the same.
The additional submission guidance is to highlight the steps for adding/removing a charter principal in the ASBCS Online System. Review if you are having trouble uploading documentation or adding/removing a change in the offices, directors, members, or partners of the charter holder.
The Charter Holder Location Amendment Request should be completed if the Charter Holder needs to make changes to its mailing address or physical address. If the Charter Holder is only making changes to its phone and/or fax numbers currently on file with Board, please contact Board Staff at 602-364-3080.
The Charter Holder Governance Amendment Request should be used if there is a change in officers, directors, members, or partners of the charter holder. These individuals are also referred to as Charter Principals. This is the only form required if the charter holder and school governing body are the same.
The Charter Holder Status Amendment Request is used to change the entity name of the charter holder, change the legal status of the charter holder, or change the ownership of the requesting charter holder to an existing charter holder with a different dashboard.

The Charter Mission Amendment Request is used to change the mission of a charter.
The Charter Representative Amendment Request should be completed if the Charter Holder needs to add or remove a Charter Representative.
The Enrollment Cap Amendment Request is used to increase or decrease the number of students the Charter Holder may serve at its school(s).
The Grade Level Change to Charter Amendment Request is used to increase or decrease the grade levels the Charter Holder is approved to serve. This request also allows for an enrollment cap increase (if needed) in one submission.
The Instructional Days Amendment Request is used to increase or decrease the number of instructional days the Charter Holder provides annually. This may be a permanent or temporary change to the charter contract.
The Member Campus Request is used for overflow situations and/or when certain grade levels are moved to another facility that is within close proximity (usually a couple of blocks but, within 5 miles) of the physical address of the school with the assigned CTDS number.
The New School Amendment Request is used to add a new charter school to the existing contract. This request also allows for an increase in grades and enrollment cap (if needed) in one submission.
The Procurement Laws Exception Amendment Request is used by the Board to grant, as permitted by A.R.S. §§15-183(E)(6) and 15-189.02, a charter holder an exception from the state procurement requirements identified in A.R.S. §§15-213 and 15-189.02. If granted, this exception applies to purchases made using non-federal funds. As a condition of the receipt of certain federal funds, federal procurement requirements still apply.
The Program of Instruction Amendment Request is used to change the methods of instruction, criteria for promotion from one level to the next, and graduation requirements. Note that any changes requested need to support the charter mission, educational philosophy, and target population as described in the charter.
The School Closure Amendment Request should be completed if the Charter Holder needs to close one of its charter schools. If the Charter Holder wants to close the only charter school operating under the contract, the Charter Holder should not use this form, but should contact Board staff at 602.364.3080.
The School Governing Body Amendment Request should be completed if there is a change to the members of the school governing body. This form should only be used if the Charter Holder and school governing body are NOT the same. If the Charter Holder and school governing body are the same, a Charter Holder Governance Amendment Request Form must be submitted.
The School Location Amendment Request should be completed if the charter holder needs to change the physical address currently on file with the Board for a charter school.
This request also allows for an enrollment cap increase (if needed) in one submission.
It is not necessary to file this request to change the school’s mailing address or phone/fax number(s) on file with the Board; rather, the authorized charter representative should log in to the ASBCS online system, choose the appropriate school, and click the “edit address” or “edit phone and fax number” link. The School Name Change Amendment Request should be completed if the Charter Holder needs to change the name of one of its charter schools.
The School Specific Change in Grades Served Amendment Request is used to increase or decrease the grades served at a particular school. This request also allows for an enrollment cap increase (if needed) in one submission.
The USFRCS Exception Amendment Request is used by the Board to grant, as permitted by A.R.S. §15-183(E)(6), a charter holder an exception from the Uniform System of Financial Records for Charter Schools, or USFRCS.
